What a lipid panel measures

A standard lipid panel reports four numbers:

  • LDL cholesterol — the “bad” cholesterol; the main treatment target for most guidelines.
  • HDL cholesterol — the “good” cholesterol; higher is generally better.
  • Triglycerides — blood fats that rise with excess calories, alcohol, and uncontrolled blood sugar.
  • Total cholesterol — the sum; less informative on its own than the components.

Many panels also calculate non-HDL cholesterol (total minus HDL), which some guidelines now emphasize.

Fasting: does it matter?

Traditionally lipid panels required 9–12 hours of fasting because triglycerides spike after meals. Many guidelines now accept non-fasting samples for routine screening, since LDL and HDL barely move — but if your triglycerides come back high on a non-fasting draw, a repeat fasting test clarifies things. Check the test instructions before your draw.

How clinicians use the results

No single cutoff decides treatment. Guidelines combine LDL with overall cardiovascular risk — age, blood pressure, diabetes, smoking, family history — often via a risk calculator. Two people with the same LDL can get different recommendations. That is why the panel is data for a conversation, not a verdict.

Retesting and tracking

Lipids respond to diet, exercise, weight change, and medication over weeks to months, which makes them good candidates for periodic rechecks. If you change your diet or start a statin, a follow-up panel in a few months shows whether it is working.

Do not start or stop medication based on a direct-access test

Statins and other lipid drugs need clinician supervision. A direct-access panel informs the discussion; it does not replace it.

Common questions

How accurate are direct-access lipid panels?

The certified labs run the same assays as doctor-ordered panels, on the same venous blood draws. LDL and HDL are directly comparable to what your doctor would order.

What is a good LDL number?

Guidelines often cite under 100 mg/dL as optimal for higher-risk people and under 130 for lower risk, but targets are individualized — your lab's reference range is the guide.

Can diet alone fix high cholesterol?

Sometimes, partly, or not much — genetics play a large role. Diet and exercise help most people somewhat; some people need medication regardless. Testing shows which camp you are in.

How often should lipids be rechecked?

Every 4–6 years for low-risk adults per some guidelines, more often if numbers are elevated or you are making changes. Retest when you make changes.
